  • You have New York State Medicaid (Medicaid Managed Care) Always Eligible
  • Food insecurity or difficulty accessing healthy meals
  • Housing instability or unsafe living conditions
  • Lack of transportation to medical or social services
  • Living with a chronic or qualifying medical condition
  • High-risk pregnancy or postpartum (up to 2 months after birth)
  • Referred by a healthcare provider, care manager, or social worker
